Community Living Arrangements/Community Residential Alternatives (CLA/CRA)

We provide CRA services for people that require intense levels of residential support in a small group setting or in host home/life sharing. Daily assistance, warm companionship helping them feel safe and independent and empowered in our CRA home. Must have Medicaid Waiver for DBHDD.

Host Homes/Life Sharing

The individual lives with a family and another individual. This service is provided in a family home for adults 21 and over. Interested in becoming a host-home? Contact us.

Structure Family Caregiver (SFC)

If you live with the family member you’re caring for and they meet the Georgia Medicaid requirements for SFC, you may qualify for these life changing benefits. If you are a Georgia family caregiver for an elderly relative, you should apply for these benefits immediately!

Advir Structure Family Caregiver (SFC)

If you live with the family member you’re caring for and they meet the Georgia Medicaid requirements for SFC, you may qualify for these life-changing benefits.

If you are a Georgia family caregiver for an elderly or disabled relative, you should apply for these benefits immediately!

Eligibility for member and caregiver

  • The person you care for needs assistance with daily living activities such as feeding, dressing, bathing, and more

  • Be Medicaid-eligible in Georgia

  • Be a participant in the CCSP/SOURCE Waiver Program

  • Must live in the same home as the caregiver.

  • Active member

Advir Community Living Support (CLS)

Must be eligible for DBHDD Medicaid Waiver (DBHDD)

We utilize the most advanced care practices available by providing our individuals with daily assistance and warm companionship, helping them feel safe, independent empowered, improvement in skills related to individual continued residence in their own homes or family home.

CLS services are provided in either the person’s own family home or own apartment/home.

Individuals that require assistance with personal hygiene, toileting, dressing, mobility, eating, light housekeeping, and other activities of daily living.

Advir Personal Care Tasks (PCT)

  • Provide bathing, toileting, and dental care

  • Provide grooming and dressing guidance

  • Provide assistance with ambulation and transfer

  • Assist in the maintenance of a safe and healthy environment

  • Provide companionship to the client

  • Prepare to serve meals and/or snacks

  • Provide emotional support and promotes a sense of well-being

  • Provide transportation and escort services

  • Participate in appropriate recreational activities and hobbies for social and sensory stimulation

  • Provide housekeeping

  • Provide laundry and linen service to include washing, ironing, and folding clothes

Advir Medical Related Task (MRT)

  • Observe and report changes in a patient’s condition

  • Arranging trips to the doctor

  • Assist with medication

  • Pickup medication and provide medication management

  • Accompany patients on medical appointment

  • Document the patient’s food or liquid intake or output

  • Ensure that all medical equipment is working in proper order
